I may not do this the justice Electro would in form of an explanation, but I’ll give it a try.
If this is not connected, each time the van is turned off, the gate will drop by gravity to the defrost position. Once the engine starts, if the hose is not leaking, the gate will move back to the position where the dash controls indicate. In essence the box holds the gate in position while the engine is off. However, if you have leak it will also drop to the defrost position because no vacuum is being held within the accumulator. (Hope I explained this right and got the right pictures).

The noise that you are hearing could possibly be the actuator trying to move the blend doors.The problem with the actuator is that the plastic teeth on the gears inside will sometimes strip away, thus not allowing it to operate correctly. If I recall correctly the blend door actuator is electrically controlled and is located behind the radio.
The issue of the air blowing only from the defrost vent, could possibly be a vacuum leak or no connection to the vacuum accumulator box under the hood. It's located on the L/H side (passenger), on the side of the air intake plenum, under the cowl. The vacuum hose is a small red plastic line that passes through the firewall and feeds to the center console.
